On this occasion I will share examples of ITP documents for piping fabrication and installation in upstream oil and gas production operations.
Purpose
This inspection and test plan is intended to be used as a guide to control all work steps in term of and inspection on piping fabrication at workshop and piping instalation at Company site location. Assure that all inspection is performed according to the approved documents and for acceptance criteria. Ensure that all parties involved have clear responsibility for all work steps.
Scope
This document covers all works step of inspection and testing on piping fabrication at workshop and piping installation at Company site location for new piping spool, to repair the existing piping system in Company facility, performed by Technical Maintenance (TM) team or Contractor (CTR) at workshop and site location.
The material scope in this ITP covers material carbon steel only.
All specific NDT procedures such as RT Procedure, UT Procedure shall be prepared by NDT Company and approved by NDT Level III personal.
When any utility engaged with Company Bussines has their own ITP and/or procedures, it shall be submitted to company for review and approval prior used.

Legend
H (Hold Point)
Work shall not be proceed without formal notification from the assigned Quality Representative from Company. A signature signifying involvement of the assigned Quality representative is required on the referenced Quality Record
W (Witness)
Personnel doing the work shall notify assigned Quality Representative from Company when the work is to be performed so that witnessing activities could take place. Work does not stop if signed Quality Representative is not present. A signature signifying involvement of the assigned quality representative is required on referenced Qaulity record.
M (Monitoring)
Verifying work and testing activities on an ongoing basis as deemed necessary, also known as patrol inspection or spot check.
R (Review)
Review of document such as procedure, drawing inspection and testing records etc, at a direction of Company.
P (Perform)
Perform or provide or prepare or execute the inspection point
A (Approved)
Defined as acceptance/approving of an plan or action after review or confirm documents.
